When your brain is abruptly unplugged from the Matrix, the brain dies.
I think an out to that rule would be if you took the person stuck in the matrix and give him/her general anesthesia to hard knock them out. No dreaming, no thinking, just sleeping.
Disconnect them, let them wake up in reality, voila.
Not as exciting as the rescue scenes with Trin, Neo and Morph, but I think it would work in reality, if this was reality.

Is it that you die because of the abrupt disconnection, or because your consciousness hasn't returned to your body.

well, if it's the consciousness, that would almost imply that it's your 'soul' or being that in the matrix as opposed to a computer rendering of the signals your brain is sending from its pod.
I am sort of assuming it's a differing input sort of thing. Who knows.

I never understood something, and perhaps, it wasn't meant
to be understood.
If the only thing that limits your power in the Matrix is your
"mind", then why wouldn't you simply hypnotize yourself
and stop your brain from processing pain and fear and
laws of physics. Everyone knows that it's possible to
stop certain brain functions through the use of hypnosis,
so if Neo hypnotized himself not to feel pain, he wouldn't
even know if he was shot because the brain would not
interpret the message the computer (Matrix) is sending
it.
It sounds weird, but I think it would work. no?

I think you are probably right. Or maybe take some mind altering drug like LSD. Might help prepare your mind to learn to ignore the reality being imposed on you.
But then again, you can't ignore all reality. Like the reality that ground is solid and I won't fall to the center of the world via gravity.
