A Matrix within a Matrix with no escape?

On my way back from the movie, one of my friends suggested
that the possibility of a Matrix within a Matrix was actually
introduced in the first Matrix with the line:

"Did you ever have a dream Neo, which you thought was so
real, what if you were unable to wake from that dream? How
would you tell the real world from the dream?" - Morpheus

One thing that didn't "CLICK" when I saw the first Matrix
is the possibility of someone escaping from the system. If
you think about it, an AI machine that controls the entire
Matrix is sure to know about someone getting unhooked.
The machine works by calculating probabilities of actions,
so it's fully aware of a human trying to escape.

Now if it let the human escape, into what the human thought
was real, then he would cease to try the escape and would
"settle in", and stop causing problems. This is why there
WAS no need for agents in Zion, but it all seems to change
with Smith.
